Step 2: Fill in the Form

 

The form is divided into 3 sections:

  • Part I (For Applicant to complete)
    Fill in your company’s name, UEN, bank details, and contact information accurately.
  • Part II (For Bank to complete)
    Forward the completed form to your bank for endorsement and signature.
  • Part III (For Singapore Customs use)
    Leave this section blank.
